Wellness by Bianca is the counselling practice of Bianca Azevedo 91 915 842 405, based in Sydney, Australia. This policy explains what personal information I collect, why, and what I do with it.
What I collect
Contact details: your name, email address and phone number.
Details you give me in forms and bookings, such as your date of birth, address, emergency contact and GP.
Health information: what you tell me about your physical and mental health in forms, messages and sessions, and my session notes.
Payment records. I do not store card details.
Website data: this site is hosted by Squarespace, which records standard visit data through cookies.
How I collect it
Directly from you, through the forms on this website, my application and intake forms, booking pages, email, WhatsApp and our sessions.
Why I collect it
To reply to enquiries, decide together whether my services suit you, provide counselling safely, manage bookings and payments, and meet my legal and professional obligations. If you subscribe, I use your name and email to send my monthly letter and event invitations. You can unsubscribe at any time using the link in every email.
Who sees it
Only me. I do not sell or share your information. I disclose it only with your consent, where the law requires it, or where there is a serious risk to your safety or someone else's.
Where it is stored
I use Squarespace (website and mailing list), Jotform (forms), Google (email, calendar and video sessions), Calendly (bookings), Canva (proposals) and WhatsApp (messages). These providers store information outside Australia, mainly in the United States. My accounts are password-protected.
How long I keep it
I keep client records for seven years after our last session, as required for health records in NSW, then delete them securely. Enquiries that do not lead to sessions are deleted after 3 months. Mailing list details are kept until you unsubscribe.
Your choices
You can ask to see the information I hold about you, or ask me to correct it, by emailing hello@wellnessbybianca.com. You may contact me without giving your full name, although I need your name and contact details to provide sessions.
Complaints
Email hello@wellnessbybianca.com and I will reply within 30 days. If you are not satisfied, you can contact the Office of the Australian Information Commissioner (oaic.gov.au) or, for health information, the Information and Privacy Commission NSW (ipc.nsw.gov.au).
